The IHC (Integrated Home Care) is a free service for users, consisting of professional medical and nursing services, of a rehabilitation and assistance type, in an integrated set of multidisciplinary treatments.
It is focused on people who need contituity care and scheduled interventions repeated over time, due to their codition.

Medical prescription.
The General Pratictioner (GP) or the Pediatrician of Free Choice (PFC) issues the user/family member with the IHC activation request, formulated on the "white and red" Regional Recipe Book (as in the photo on the side), indicating the diagnosis and the specific request for home care.
It is your right to choose J-care srl, ust call the telephone number of our Service Center (02-821801) or fill in this link to agree on taking care of the patient.
Who can access the service?
Integrated Home Care is dedicated to frail or disabled people if:

they are in a situation of non-self-sufficiency, partial or total, temporary or definitive;

they are unable to walk or in a non-transportable condition at outpatient healthcare facilities;

they have a family and/or formal or informal support network;
-
they are in housing conditions that guarantee the practicability of assistance.
